Delayed transfer of care is when a patient is ready to leave a hospital or similar care provider but has not been able to be discharged. These delays can occur when patients are being discharged home, to a residential or nursing home for example, or are awaiting transfer to a community hospital or hospice. They usually are a result of waiting for assessments to be completed or arrangements for social care support or residential placements.
